Support Embrace Connect

What We Do

We provide practical, person-centred disability support that helps participants build independence, strengthen confidence and stay connected with home, family and community.

Support that helps participants live well

Support Embrace Connect works with participants, families, carers, and support coordinators to provide flexible, respectful, and goal-focused services. Our supports are designed to help people take part in daily life, develop meaningful skills, and feel supported in the environments that matter most to them.

Core Supports

Everyday support that helps participants access the community, manage daily routines and feel more confident at home.

Community Support

We support participants to access their local community, attend appointments, take part in social activities, complete errands and enjoy meaningful connection outside the home.

In-home Support

We provide practical assistance with daily living routines, home organisation, meal preparation, household tasks and confidence-building support that suits the participant’s needs.

Respite and Flexible Support

We can assist with flexible support arrangements that help participants and families maintain routine, wellbeing and stability while working toward NDIS goals.

Capacity Building Supports

The SEC team can support participants with decision-making, planning, and budgeting.

Independent Skill Building

We assist participants to develop everyday skills such as cooking, planning, budgeting, public transport confidence, communication and self-management routines.

Increased Social and Community Participation

SEC supports provide a safe and respectful space to talk through challenges, build coping strategies, and strengthen emotional well-being.

Assistance with Decision Making, Daily Planning and Budgeting

e can help participants understand their NDIS plan, connect with their LAC or Support Coordinator to help coordinate necessary supports, and stay focused on their goals.

Community and Social Programs

Structured and flexible activities that support connection, skill development and positive participation.

Social Outings and Activities

We support participants to take part in recreational activities, social outings, special events, team building, art, cooking, gardening and other activities that encourage participation and connection.

Goal-focused Community Access

Community access can be tailored around each participant’s plan goals, interests and confidence level, with support that encourages independence and safe participation.
